What Is Strep Throat?
Strep throat is an infection caused by the bacteria called group A streptococcus. Unlike many sore throats caused by viruses, strep throat typically does not go away on its own without treatment. Common signs of strep throat include:
- Sudden, painful sore throat,
- Fever, especially without cold symptoms,
- Swollen glands (lymph nodes) in the neck,
- White patches or redness in the throat,
- Pain when swallowing,
- A sore throat that lasts more than a few days or doesn’t respond to over-the-counter medications and treatments.
Sometimes, these symptoms occur with a rash, a red, bumpy tongue (known as “strawberry tongue”), and chills or body aches. Then, it might be scarlet fever, another infection caused by the same bacteria that cause strep throat.
These infections are most common in children, but adults can get them, too. Because symptoms can overlap with viral infections, such as the flu, testing is the only way to know for sure.

Why Strep Throat Testing Is Important
Strep throat is a bacterial infection, which means it needs prescription antibiotics to:
- Shorten how long symptoms last,
- Prevent spreading it to others,
- And reduce the risk of complications like rheumatic fever or kidney problems.
Antibiotics do not help viral sore throats. Taking them when they aren’t needed can cause side effects and antibiotic resistance. That’s why testing first is so important; it allows us to give the right treatment, not just any treatment.
Furthermore, although adults should get seen for these symptoms, strep throat testing and treatment are crucial for children. They are much more vulnerable to complications, such as rheumatic fever and kidney problems. These chronic conditions, although rare, can even lead to death.
That’s why, if you or your children exhibit any of the symptoms above, it’s crucial to make an appointment sooner rather than later.
How Providers Test for Strep Throat
Strep throat testing is quick and simple. It usually involves a gentle swab of the throat. For those with a sensitive gag reflex or a very sore throat, this can be uncomfortable. However, we know it’s unpleasant, so we are very quick. And any irritation from the swab will go away quickly.
There are two main types of tests:
- Rapid strep test: Provides results on the same day as your appointment, often within minutes.
- Throat culture: Used when a rapid test is negative but symptoms strongly suggest strep, especially in children. Results take longer but are even more accurate.
Most people only need the rapid strep test, especially if it comes out positive. From there, we know what antibiotics to prescribe and how to help you feel better within days.

What Happens if Strep Throat Testing Is Positive
If your strep throat test comes back positive, your provider will prescribe antibiotics to treat the infection. These medications help kill the bacteria, ease symptoms more quickly, and reduce the risk of complications and spreading strep to others.
Most people start to feel better within 24 to 48 hours after starting antibiotics. Fever often decreases first, followed by reduced throat pain and easier swallowing.
You are usually no longer contagious after 24 hours of antibiotics and once your fever is gone.
That said, it is very important to take the full course of medication, even if you start feeling better, to make sure the infection is fully treated.
What If Strep Throat Testing Is Negative?
If your strep test is negative, the next steps depend on age and symptoms.
In Children
- A throat culture may be done to confirm a negative result while testing for other bacterial infections.
- Providers may evaluate for mono if symptoms are severe or long-lasting.
If all tests are negative, the cause is usually viral or allergy-related, and we will provide supportive care for symptoms while asking parents to monitor any lingering or worsening signs.
In Adults
- Testing for flu or COVID-19 may be appropriate.
- If these are also negative, symptoms may be related to other causes. We may offer allergy testing and assess for issues such as post-nasal drip or acid reflux, especially if you experience frequent sore throats.
A negative strep test doesn’t mean nothing is wrong; it simply helps us look for the right cause.

How to Ease Sore Throat Symptoms at Home
While antibiotics treat the cause of strep throat and other bacterial infections, there are several things you can do at home to help ease discomfort while you recover:
Drink plenty of fluids to keep your throat moist.
Gargle with warm salt water to reduce irritation.
Use throat lozenges or hard candy (for older children and adults).
Take over-the-counter pain relievers as directed.
Rest your voice and get plenty of sleep.
Warm liquids like soup or tea can be soothing, and a cool-mist humidifier may help if the air is dry. These steps won’t cure the infection, but they can make you more comfortable as your body heals.
